To add on the localization thing, unfaithful localizations injected with Western politics blew up recently in Japan to the point some Japanese production companies are having people fired and switching to AI/Human hybrid translation because they and Japanese audiences find it disrespectful to change a translation that much. Japanese culture and politics just doesn't have the same kind of focus on gender identity that the Western left does which is why the audiences of there don't really find femboys and crossdressing offensive when it's portrayed in anime, manga, and games. Yes LGB rights are an issue in Japanese, but it's a lot closer to what things were like before the trannies and gender spergs infected the LGBT movement in the English speaking world. Western Leftards often forget or ignore that Japan and a lot of non-English speaking countries are relatively conservative in both politics and culture. It's why stuff like that US tranny that tried to force his way into a Japanese lesbian bar got shot down very quickly and refused service and why Japan is very not tolerating of Western youtubers causing trouble for e-fame.

It's not a new thing either since it has origins from when kabuki theater had men commonly playing the women roles too. First otokonoko anime/manga character I recall seeing was Konatsu from Ranma 1/2, so even anime and manga it's been a character trope for decades. In Japanese media they usually are still attracted to women, at best teasing other men with their appearance and behavior for comedy. It's a pretty common joke for these characters to either act offended when an unknowing male character starts hitting on them thinking they're a girl or for the guy hitting on them to comically freak out upon finding out their actual sex. Closest Western counterpart I can think to make for otokonoko in Japanese media is crossdressing being used for comedy such as Some Like It Hot, Bosom Buddies, Monty Python skits, Big Momma's House, and the Madea films.
