Like other anons have said, Venom probably got facial surgery while in a coma. If not that, dude just kept his hair/beard unkept and wore an eyepatch and that was have enough to convince people he's BiBo.
Emphasis on 'probably' here. A lot of things in MGSV are either only briefly explained or left to interpretation, the reasoning for which is the implication of Venom having schizophrenia/hallucinations from brain damage.
MGSV's big theme, aside from revenge, is about chasing phantoms -
>Miller's obsession with hunting Cipher/Zero, whom Skull Face already took down in between GZ and TPP (If i remember correctly)
>Venom delivering photographs to Paz to try and jolt her memory (before she's revealed to be entirely a hallucination)
>Volgin chasing Venom (before he discovers Venom's true identity in a side mission and gives up on living)
>There's also a bunch of Moby Dick references to reinforce this (Ahab, Ishmael, Queequeg, the bigass fire whale that swallows the chopper during the hospital escape)
I'm convinced Kojima used the theme of phantoms as an excuse to cover his ass and insert as much stupid shit as he wanted (granted he'd been working on Metal Gear the majority of his time at Konami and probably stopped giving a shit, but still).
Every weird, contradictory, or out-of-left-field story beat can be explained away with "hurr durr Venom hallucinates pls understand", to the point where whether or not the game even happened can be questioned without much difficulty at all.
Despite MGSV being touted as 'the missing link between MGS3/PW and MG1, nothing of actual narrative importance affecting the rest of the games happens in TPP, either.
The only thing I can think of/remember which COULD explain things being the way they are in other titles is

>The truth tapes re-contextualizing Zero's actions after MGS3 (the best part of TPP imo)
Basically, whether or not Venom's face is (you)r avatar's or BiBo's doesn't matter because almost nothing else that happens in this game does either.
